Who qualifies for home care?

An individual with a skilled need (i.e., wound care; dressing changes; physical or occupational therapy; disease process and health care teaching, teaching regarding new medications; intravenous medication therapy). Home care provides a wide range of services and it is best to discuss with your doctor if you or your loved one qualifies for home care. You may also contact us and we can help you determine if you may be eligible for services.

Who qualifies for hospice care?

Any individual with a terminal illness may be eligible for hospice care if they are no longer pursuing treatment or a cure.

What services are provided for home care and hospice? 

From newborns to the elderly, we provide a wide range of care and services. In fact, many treatments that just a few years ago were done only in an institution can now be provided in the home. Our professional staff of caregivers includes:

  • Skilled Nurses
  • Certified Home Health Aides, Certified Nursing Assistants (VNA) and Personal Care Attendants (VNA)
  • Licensed Medical Social Workers
  • Physical, Occupational and Speech Therapists
  • Specific to hospice: We assist patients and families in quality end-of-life care by offering the following: Chaplain, nutrition, pain management, volunteers and bereavement services. For a complete list please contact us.
How do I get a referral for home care?

We accept referrals from hospitals (if you are being discharged from the hospital), your physician or you or your family member. We must have a physician’s order to admit you to home care services.

How do I get a referral for hospice care?

A patient, patient’s doctor or family member may make a referral to hospice care. If you believe you or a loved one is eligible for these services based on the above criteria, you can contact us and request an informing visit. A hospice representative will come directly to you to determine your eligibility for the program.

Who pays for home or hospice care?

Most Medicare, Medicaid and private pay insurers will pay for home care or hospice services if individuals meet their criteria. Our intake departments can help you with this information to determine if you or your loved one is eligible for care. Your particular insurance may not cover certain services, so it’s important to work with your doctor’s office or speak with our home care agency intake department to determine what services for which you are eligible.
